If you were a client of Daryl J Petry, Inc., welcome. These are the forms from my welcome letter: a quick reply about your final return meeting, and the §7216 consent forms that let your records come to NTM CPA. Fill in the reply form right here, or download any of them as a fillable PDF (they're also attached to my welcome email).

How and when should we meet?

Your final return meeting is when we review your finished return together before it is filed, between February and April 2027. It's separate from any first call we've already set up. Tell me how you'd like to meet and roughly when (or pick “Not sure yet”), and I'll follow up to set the exact day and time. Please don't include Social Security numbers or tax documents here; I'll collect those securely through my client portal.

§7216 consent forms

Bringing your records to NTM CPA.

Federal law (Internal Revenue Code §7216) requires your written permission before your tax return information can be disclosed to another preparer or used for anything beyond preparing your return. There are two short forms, one for each permission. Signing them is voluntary, and never a condition of my services.

Form 1

Consent to Disclose

Permission for Daryl J Petry, Inc., your prior tax return preparer, to disclose your prior income tax returns, with their supporting schedules and statements, to Noblesville Tax Management, LLC so I can provide your future tax and accounting services. Valid for one year from the date you sign.

Form 2

Consent to Use

Permission for Noblesville Tax Management, LLC to use the information in your tax return to prepare your future income tax returns. Valid for one year from the date you sign.

Prefer to sign electronically? Tick “Please email me (and my spouse) the §7216 consent forms” in the reply form above, and I'll send each consent through Dropbox Sign. If you file jointly, you each sign from your own email.

On paper

Print and sign

Fill in the PDF (attached to my welcome email, or download it here) with your printed name and the date, then print it and sign in ink. If you file jointly, you both sign the same form. Email a scan or phone photo to michael@ntmcpa.com, or mail it to 14350 Mundy Drive, Suite 800-290, Noblesville, IN 46060. Typed or pasted signatures can't be accepted on these forms.
